GINGIVITIS
 Gingivitis is an inflammation of the gingiva (gums), characterised by redness, swelling, bleeding  easily when brushed or prodded, tenderness and usually bad breath. Swelling deepens the crevice between gums and teeth, where pockets containing pus may develop to cause a condition called pyorrhoea. Gingivitis is common and may be acute, chronic or recurrent.
 The most frequent single cause of gingivitis is poor dental hygiene with a build-up of dental plaque around the base of the teeth. Other localised causes include tartar, calcified plaque, food traps between teeth and ill-fitting dental plates, caps or crowns. The gum inflammation that often develops during puberty or pregnancy is thought to be aggravated by hormonal factors. Gingivitis may also be a symptom of general illness for example in Diabetes mellitus and leukaemia and malnutrition especially in scurvy and pellagra Vitamin B3 deficiency.
 Gingivitis is treated by control of plaque and correction of any other local or general conditions that contribute to the disorder.  Good dental hygiene with regular tooth brushing and use of dental floss will prevent most gingivitis. Acute ulcerative necrotizing gingivitis is a non contagious, painful, ulcerating infection of the gums also called trench mouth or Vincents disease. This can develop in heavy smokers or people suffering poor dental hygiene, nutritional deficiencies or physical or emotional stress.  The mouth has an extremely bad odour, and the victim may develop a fever. The plaque and calculus irritate the gums, causing them to become inflamed.  In time, the bone supporting the teeth may become inflamed.  The best way to prevent plaque from building up under the gum line is by flossing daily.  The gums can also become irritated by habitually breathing through the mouth, smoking or chewing tobacco, brushing improperly, or wearing ill-fitting dentures.  Irregularly positioned teeth can also irritate the gums. 
 Symptoms develop rapidly over a day or so and include very painful bleeding gums, foul breath, crater like ulcers covered with a grayish membrane particularly on the points of gums between the teeth and usually swollen glands around the jaws.  People who smoke or chew tobacco or drink excessive amounts of alcoholic beverages are exposing themselves to an increased risk of developing oral cancer.  Oral cancer may be painless and unnoticeable in its early stages.  The first symptom may be a small sore in the mouth that does not heal.

GALL STONES
Solid bodies that form in the gall bladder from constituents of bile such as cholesterol, bile pigment, mucin and calcium.  Gall stones a small sac found just under the liver. It stores bile made by liver. Bile helps to digest fats. Bile moves from gallbladder to small intestine through tubes called cystic duct and common bile duct. The most common symptom is pain in stomach area or in the upper right part of the belly, under the ribs.The size of gall stones ranges from  grit like to several centimetres in width.  There may be a single stone but more often there are many.
CAUSE: The cause of gall stones, also called cholelithiasis is uncertain. It has been suggested that bile containing more than the usual amount of cholesterol, a normal bile constituent and a smaller than usual amount of bile acid may be likely to lead to stone formation. Production of this type of bile is an inherited tendency, which would explain why gall stones often run in families. Other possible factors include inflammation or infection of gall bladder, increased bile pigment formation and hormonal and other genetic factors.
INCIDENCE: Gall stones  are common in men and women.
PAIN: It may be develop suddenly in center of upper belly and spread to right upper back or shoulder-blade area. It is usually hard to get comfortable; moving around does not make pain go away.  It also prevent from taking normal or deep breaths. Normally it begins at night and be severe enough to wake you and mostly occur after meals. There are many other conditions that cause similar symptoms,including heartburn, pain caused by heart attack, and liver problems. Pancreatitis: Pancreatitis is an inflammation of pancreas, which is an organ in the upper abdomen that makes insulin and digestive enzymes. Pancreatitis may cause sudden, severe abdominal pain. It is most commonly caused by excessive use of alcohol or by blockage of tube(duct) that leads from pancreas to beginning of small intestine(duodenum),usually by a gall stone. Other causes include an infection,injury, or certain medicines. It may develop suddenly(acute),or it may be a long-term,recurring(chronic) problem, but may develop complications such as bleeding,infection,or organ failure may develop. The pancreas is an organ in upper belly, behind stomach and close to spine. It makes digestive enzymes that body uses to break down and process food. The pancreas also makes insulin, hormone that regulates sugar (glucose) in the blood.
SYMPTOMS: The most common symptoms are bouts of moderate upper abdominal pain or in upper right part of belly, sometimes brought on by a fatty meal. Pain may spread to right upper back or shoulder-blade area. Sometimes pain is more severe. It may be steady, or it may come and go. Or it may get worse even after eating something heavy.  When gall stones keep blocking bile duct,there is a pain with fever and chills, or the skin or whites of your eyes may turn yellow. Having stones in bile duct increases the chance of having swollen pancreas (pancreatitis). These symptoms may also be a sign of an infected gallbladder. A stone wedged in the ducts that drain the gall bladder causes the agonising pain of biliary COLIC.  If the stone blocks the common bile duct which drains bile from both the liver and gall bladder there may be jaundice as well as colic. BREAST CANCER
• In its early stages, usually it does not have any symptoms.  The diagnosis of breast cancer has a profound emotional impact because of fears and it affects a part of the body intimately linked with a woman’s self image and sexuality. As a tumor develops with the following signs :
• A thickening or lump in the breast or armpit usually observed after the menstrual cycle; often the first apparent symptom of breast cancer, breast lumps are painless, although some may cause a prickly sensation. Lumps are usually visible on a mammogram long before they can be seen or felt.
• Swelling in the armpit.
• Although lumps are usually painless, pain or tenderness in the breast can be a sign of breast cancer.
• A noticeable flattening or indentation on the breast, which may indicate a tumor that cannot be seen or felt.
• Any change in the size, shape, contour, texture, or temperature of the breast, like dimple in the skin, a reddish, pitted surface like the skin of an orange could be a sign of breast cancer.
• A change in the nipple, such as an indrawn or dimpled look, itching or burning sensation, or ulceration; scaling of the nipple is symptomatic of Paget’s disease, a localized cancer.
• Unusual discharge from the nipple that may be clear, bloody, or another color. It’s usually caused by benign conditions but could be due to cancer in some cases.
• Change in the color or peel of the skin around the nipple.
• A marble-like area under the skin.
• Dilated veins.
• Hardness of the breast.
• An area that is distinctly different from any other area on either breast. One or both breasts develop an abnormal lump or persistent pain, or look or feel abnormal. The cause often is something other than cancer but should be identified.
• There may be swollen lymph glands in the armpits. Any such swelling could be associated with cancer.
It begins with the formation of a tumor. If the tumor is malignant, it has the potential to grow and spread or metastasize to different parts of the body through the blood stream or the lymphatic system.  Almost all breast cancers begin in the glandular structures in the breast that, during lactation, produce milk. These mammary glands are under the control of reproductive hormones that stimulate the monthly cycle of gland expansion and shrinkage, which is a feature of the regular menstrual cycle. Many of the factors associated with the development of breast cancer appear to have their effect through interaction with the hormonal stimulation of these glands. It can be ‘invasive’ (spreading) or ‘noninvasive’ (do not spread and is confined to the original site). There are different kinds of breast cancer;
 Ductal carcinoma in situ(DCIS),  Lobular carcinoma in situ(LCIS),  Invasive(infiltrating)ductal carcinoma(IDC): This kind of cancer begins in the milk passage, breaks through the wall of the passage and spreads in the breast tissue. Later the cancer cells spreads to different parts of the body. Invasive (infiltrating) lobular carcinoma (ILC): This condition begins in the milk lobules and spreads to different parts of the body.
 Inflammatory breast cancer(IBC): This is a rare invasive condition where a tumor or lump is not seen,instead the skin of the breast looks red and warm to feel. The skin is thick with pitted appearance and looks like an orange peel. The breast becomes larger, firmer, tender and itchy.
 Diagnosis is based on detection and screening. It can be done through self-examination of the breast frequently and finding out for the presence of lumps, pain in the breast, thickness, changes during menstruation. Getting checked with a physician is necessary if anything unusual is observed. Self-examination helps in early diagnosis of the disease.
 But if one has already have the Chemotherapy, it may develop some side effects which causes nausea, vomiting, infertility problems and early menopause by destroying the eggs in the ovaries.
 Hormonal therapy causes vaginal dryness and hot flashes after menopause. Hormonal therapy: Those who have undergone menopause and taken hormone therapy for more than 4 years with estrogen and progesterone have high chances.
Herceptin can cause heart problems by weakening the heart muscles.
Side-effects of radiation therapy include swelling in the breast, reddening and blistering of the skin and fatigue.
One should avoid long-term hormone intake after menopause.  Breast-feeding will lower the risk.  Conceiving before 30 years of age.  Loosing extra kilos, as extra fat increases estrogen production.  Consuming healthy food with plenty of fruits, vegetables and whole grains.  Exercising regularly.  Decrease alcohol consumption.
The chances are high as the age increases or if anybody in the family has suffered from breast cancer indicating genes are responsible. If one has breast cancer in one of the breast, there is a likelihood of getting affected to the other breast too. Chances of recurrences are high to the same part or to different parts of the body.
Menstruation: Women with menstruation beginning before the age of 12 years and menopause occurring after the age of 55 years are at high-risk of getting affected with breast cancer.
Having higher estrogen levels increases the risk.

The main source of pain and stiffness is the protective spasm of muscles around the injured part. When back pain is the result of a minor strain, it can overcome very effectively in a short span of time. The treatment for acute symptoms of uncomplicated back strain consists of rest. If one feels pain while on rest or if it is severe enough to restrict most activities or there is no improvement in a week’s time, it needs more investigation. Chronic back pain associated with other symptoms such as fever should always be investigated as it may be a sign of disease of the vertebrae or spinal joints and occasionally may be caused by disorders of the abdominal or pelvic organs particularly the kidneys and reproductive organs.

One should follow the followings when driving long distances;
 • Travel slowly over rough roads.
 • Ensure that the vehicle has well padded seats, which will help reduce the amount of vibration being passed up the spine.
 • Support the lower back with a small cushion or lumbar roll.
 • Tilt the seat backwards to about 110 angel to reduce vertical pressure on the spine.
 • One should change the posture frequently to encourage circulation and prevent muscle tiredness.
 • Make sure that the distance from the seat to the foot pedals suits your legs.
 • Take heavy objects out of back pockets to reduce twisting forces on the lower spine.
 • Take care when getting in and out of vehicles. Sit sideways on the seat first then lift your legs in and rotate your body into the travelling position and do the opposite when getting out. Avoid sudden bending or twisting movements.
 • Reduce driving time. Frequent stops help. As often as possible get out, stretch your back and walk around a little.

 Reducing back strain;
 
-When lifting heavy objects, keep your back straight and  bend your knees. Do not stoop, if the object is too heavy, ask someone to help.
 -At the desk, keep your back straight and rest both feet on the floor. A well designed office chair provides good back support.
 -Your load should be equally distributed if possible when you are carrying bags.
