BREAST CANCER


BREAST CANCER
• In its early stages, usually it does not have any symptoms.  The diagnosis of breast cancer has a profound emotional impact because of fears and it affects a part of the body intimately linked with a woman’s self image and sexuality. As a tumor develops with the following signs :
• A thickening or lump in the breast or armpit usually observed after the menstrual cycle; often the first apparent symptom of breast cancer, breast lumps are painless, although some may cause a prickly sensation. Lumps are usually visible on a mammogram long before they can be seen or felt.
• Swelling in the armpit.
• Although lumps are usually painless, pain or tenderness in the breast can be a sign of breast cancer.
• A noticeable flattening or indentation on the breast, which may indicate a tumor that cannot be seen or felt.
• Any change in the size, shape, contour, texture, or temperature of the breast, like dimple in the skin, a reddish, pitted surface like the skin of an orange could be a sign of breast cancer.
• A change in the nipple, such as an indrawn or dimpled look, itching or burning sensation, or ulceration; scaling of the nipple is symptomatic of Paget’s disease, a localized cancer.
• Unusual discharge from the nipple that may be clear, bloody, or another color. It’s usually caused by benign conditions but could be due to cancer in some cases.
• Change in the color or peel of the skin around the nipple.
• A marble-like area under the skin.
• Dilated veins.
• Hardness of the breast.
• An area that is distinctly different from any other area on either breast. One or both breasts develop an abnormal lump or persistent pain, or look or feel abnormal. The cause often is something other than cancer but should be identified.
• There may be swollen lymph glands in the armpits. Any such swelling could be associated with cancer.
It begins with the formation of a tumor. If the tumor is malignant, it has the potential to grow and spread or metastasize to different parts of the body through the blood stream or the lymphatic system.  Almost all breast cancers begin in the glandular structures in the breast that, during lactation, produce milk. These mammary glands are under the control of reproductive hormones that stimulate the monthly cycle of gland expansion and shrinkage, which is a feature of the regular menstrual cycle. Many of the factors associated with the development of breast cancer appear to have their effect through interaction with the hormonal stimulation of these glands. It can be ‘invasive’ (spreading) or ‘noninvasive’ (do not spread and is confined to the original site). There are different kinds of breast cancer;
 Ductal carcinoma in situ(DCIS),  Lobular carcinoma in situ(LCIS),  Invasive(infiltrating)ductal carcinoma(IDC): This kind of cancer begins in the milk passage, breaks through the wall of the passage and spreads in the breast tissue. Later the cancer cells spreads to different parts of the body. Invasive (infiltrating) lobular carcinoma (ILC): This condition begins in the milk lobules and spreads to different parts of the body.
 Inflammatory breast cancer(IBC): This is a rare invasive condition where a tumor or lump is not seen,instead the skin of the breast looks red and warm to feel. The skin is thick with pitted appearance and looks like an orange peel. The breast becomes larger, firmer, tender and itchy.
 Diagnosis is based on detection and screening. It can be done through self-examination of the breast frequently and finding out for the presence of lumps, pain in the breast, thickness, changes during menstruation. Getting checked with a physician is necessary if anything unusual is observed. Self-examination helps in early diagnosis of the disease.
 But if one has already have the Chemotherapy, it may develop some side effects which causes nausea, vomiting, infertility problems and early menopause by destroying the eggs in the ovaries.
 Hormonal therapy causes vaginal dryness and hot flashes after menopause. Hormonal therapy: Those who have undergone menopause and taken hormone therapy for more than 4 years with estrogen and progesterone have high chances.
Herceptin can cause heart problems by weakening the heart muscles.
Side-effects of radiation therapy include swelling in the breast, reddening and blistering of the skin and fatigue.
One should avoid long-term hormone intake after menopause.  Breast-feeding will lower the risk.  Conceiving before 30 years of age.  Loosing extra kilos, as extra fat increases estrogen production.  Consuming healthy food with plenty of fruits, vegetables and whole grains.  Exercising regularly.  Decrease alcohol consumption.
The chances are high as the age increases or if anybody in the family has suffered from breast cancer indicating genes are responsible. If one has breast cancer in one of the breast, there is a likelihood of getting affected to the other breast too. Chances of recurrences are high to the same part or to different parts of the body.
Menstruation: Women with menstruation beginning before the age of 12 years and menopause occurring after the age of 55 years are at high-risk of getting affected with breast cancer.
Having higher estrogen levels increases the risk.
More fat in the body and consuming alcohol increases estrogen levels especially after menopause leading to an increased risk. It has been suggested that a diet  high in fat and or sugar may be causative and that high fibre intake may be protective, but these theories have not been proven.
Radiation therapy: Young women who have exposed their breasts to radiations due to some diseased condition like Hodgkin’s lymphoma are likely to develop breast cancer in the later ages.
Not breast-feeding: Women who never breast-feed their children are at high risk.
The risk for breast cancer is also significantly decreased among women undergoing surgical removal of the ovaries, particularly if the surgery is performed before age thirty-five,as removes the major source of reproductive hormones and therefore results in less stimulation of the glands in the breast.
Conversely, the greater number of years a woman has regular menstrual cycles, the higher the risk of breast cancer. There is also a modest increase in risk associated with postmenopausal estrogen replacement therapy (especially when used more than 15 years), and with exposure to the synthetic estrogen diethylstilbestrol during pregnancy. Studies have found a significant correlation between breast cancer and levels of hormones—estradiol, estrone, estrone sulfate, prolactin, and dehydroepiandrosterone sulfate. A drug used to treat breast cancer, tamoxifen, blocks estrogen receptors.
Taken together, a significant body of research shows that reproductive hormones—produced internally and taken as medicines—are major determinants of breast cancer risk. Other factors—including genetic predisposition, environmental exposure, and lifestyle choices—may increase cancer risk via hormone regulation.

The high level of urbanization and intensity of production processes has impact on all spheres of human life. HORMONES 
 The nervous system control body activities through electrical impulses, the endocrine system uses chemical mesengers called hormones. Both the systems combined control all unconscious body functions and maintain our body at the best.  The endocrine glands release hormones that are absorbed into the blood so that they circulate and can act on one, or more than one, distance tissue.  Chemical receptors on the surface of or inside the target cells respond to each hormone, resulting in specific cell activities.  The hypothalamus, part of the brain, links nervous and hormonal controls mainly by its influence on the nearby pituitary gland, which in turn regulates the activity of the thyroid gland, adrenal glands and gonads (ovaries and testes). The other  principal endcrine glands are the parathyroids and the insulin-producing islets of the pancreas.
 The level of all hormone secretion is delicately adjusted to avoid too much or too little, mainly by feedback control.  When cheical sensors in the hypothalamus detect that thyroid hormones in the blood are too low, the hypothalamus directs the pituitary to release more thyroid stimulating hormone so that more thyroid hormone is produced. When blood thyroid hormones becoe adequate  this extra stimulation stops. 
 The endocrine glands ;
 Pituitary ; Lying at the base of the brain beneath the hypothalamus, it has been called the master gland as it regulates body activities. It has two parts, the anterior(front) and posterior(back). Anterior pituitary hormones stimulate endocrine or other glands to produce secretion or grow, in addition it stimulate the thyroid, adrenals and gonads. Its growth increases the rate of growth of all tissues andprolactin stimulates milk production. The posterior pituitary produces oxytocin, to stimulate uterine contractions in childbirth, and antidiuretic hormone, which regulates the volume of urine produced.
 Thyroid ; This gland is wrapped around the front of the trachea, windpipe and its hormones regulate the metabolism and growth by increasing oxygen consumpton, energy production and protein synthesis. The thyroid needs iodine from food and water to make its main hormones, thyroxine(T4) and tri-iodothronine(T3). A third hormone, calcitonin, is produced by different thyroid cells and helps parathyroid hormone to regulate calcium levels in the blood.
 Parathyroid Glands ; When chemical sensors in the four pea sized parathyroid glands behind the thyroid detect that blood calcium is too low, they secrete more calcium being released from bones into blood, more being absorbed from the intestine and more being conserved by the kidneys. The action quickly restore the blood calcium level, if it begins to get too high, the parathyroids release less hormone.
 Adrenal Glands ; One capping each kidney, the pyramid shaped, adrenal glands consist of two parts, an outer cortex and an inner medulla. The cortex produces a number of corticosteroid hormones; aldosterone, which helps to maintain adequate body water by influencing reabsorption of minerals by the kidneys; glucocorticoids which promote glucose production and help the body respond to stresses such as injury and severe anxiety; and some male hormones in both the sexes. The adrenal medulla is part of the sympathetic division of the autonomic nervous system and produce adrenalin and noradrenalin inresponse to acute physical or emotional stress.  Impulses from the hypothalamus travel along sympathetic nerves to jolt the adrenal medulla into releasing its hormones to prepare the body for immediate protective action.
 Pancreas ; Scattered throughout the pancrease enqyme producing tissue are small clusters of endocrine cells, the islets of langerhans. Different cells in the islets produce hormones, insulin and glucagon, which regulate fluctuations in blood glucose levels. When blood glucose increase after a meal, insulin limits its level by speeding the entry of glucose into the cells and by accelerating conversion of glucose to glycogen for storage. Glucagon has the opposite effect; when blood glucose falls below normal, it hastens conversion of glycogen to glucose and helps the liver to convert oter nutrients into glucose.
 Testes, Ovaries : Inside the testis-connective tissue partitions divide the testis into lobules that house the coiled tubules, from which fully formed sperm travel via short ducts to the epididymis.  AND, closs section of shaft of penis contains three cylinders of erectile tissue surrounded by capsules – the two cavernous bodies in front and a spongy body behind. During sexual arousal blood distends erectile tissue and makes it hard. With its thinner capsule, the spongy body becomes less hard so that semen can be ejaculated through the urethra.  Reproductive system, controlled by Coldness meridien ; Uterus is a hollow organ as Yang, so is penis that becomes fulled with the blood. Both, uterus and penis are controlled by the Spinal Cord.  Ovaries and testies are full of substance and controlled by the brain meridien.
