HYPOCHONDRIA
Hypochondria are a belief that symptoms are signs of a serious illness, even though there is no medical evidence to support the presence of an illness, also known as Hypochondriasis, a type of somatoform disorder, mental illness in which a person has symptoms of a medical illness, but the symptoms cannot be fully explained by an actual physical disorder.
Such people are very much worried about getting a disease or are sure they have a disease, even after medical tests show they do not have any such disease. Further, these people often misinterpret minor health problems or normal body functions as symptoms of a serious disease. The symptoms are not under the person’s voluntary control, and can cause great distress and/or can interfere with a person’s normal functioning.
As such people excessively focus on the self-health, they have an unrealistic fear of having a serious disease. People with hypochondria are not in a place to control their fears and worries. The way people with hypochondria think about their physical symptoms can make them more likely to have this condition. As they focus on and worry about physical sensations, a cycle of symptoms and worry begins, which can be difficult to stop.
It is important to realize that people with hypochondria do not purposely create these symptoms (malingering). They are unable to control the symptoms.
The symptoms they describe can range from general complaints, such as pain or tiredness, to concerns about normal body functions, such as breathing or stomach noises. People with hypochondria are not faking or lying about their symptoms; they truly believe they are sick.
The signs that a person might have hypochondria include:
•The person has a history of going to many doctors. S/he may even “shop around” for a doctor who will agree that he or she has a serious illness.
•The person recently experienced a loss or stressful event.
•The person is overly concerned about a specific organ or body system.
•The person’s symptoms or area of concern might shift or change.
•A doctor’s reassurance does not calm the person’s fears; they believe the doctor is wrong or made a mistake.
•The person’s concern about illness interferes with his or her work, family, and social life.
•The person may suffer from anxiety, nervousness, and/or depression.
People who have a history of physical or sexual abuse are more likely to have this disorder. However, this does not mean that every person with a hypochondria disorder has a history of abuse.
They seek out reassurance from family, friends, or health care providers. They feel better for a short time at most, and then begin to worry about the same symptoms, or about new symptoms.
Symptoms may shift and change, and are often vague. Such people often look at their own body.
Those who are affected may recognize that their fear of having a serious disease is unreasonable or unfounded.
Expectations (prognosis)
The disorder is usually long-term (chronic), unless psychological factors or mood and anxiety disorders are treated.

 A sensation of light-headedness and unsteadiness, is often caused by a decrease in blood supply to the brain, while vertigo is caused by disturbances in the inner ear and the balance centers of the brain. It is necessary to understand the specific symptoms a patient is experiencing in order to make the correct approach;
Common causes of dizziness –
Low blood pressure – Dizziness, light headedness, and the feeling of passing out is a common complaints who have low blood pressure. When the blood pressure is too low, not enough oxygen-rich blood is being delivered to the brain, and its function can be affected.

•nausea and vomiting,
•shortness of breath, and
•chest pain.

•dehydration (loss of water in the body) often occurs with infections that cause vomiting and diarrhea.
Most attacks of dizziness are brief and harmless and are caused by a momentary reduction of the blood supply to the brian,such as may occur when getting up suddenly after sitting or lying down. However, dizziness may be a symptom of ear or brain disorders, cardiovascular disease, anaemia or Hypo or Hyper – glycemic or it may be a side effect of certain drugs – especially those used to treat high blood pressure.

•addison’s disease: it occurs when the adrenal glands do not produce enough cortisol to meet the needs of the body, it’s a naturally occurring steroid produced by the body and is part of the stress response (fight or flight). If cortisol levels are low, a patient may experience weakness, fatigue, dizziness, light-headedness, low blood sugar, and low blood pressure.
Vertigo – Vertigo is often described as feeling dizzy, or a sensation of spinning. The patient may specifically mention that the room seems to be spinning around them. Sometimes the complaint is loss of balance or loss of equilibrium. This often occurs because of irritation in the inner ear (the part of the ear that involves balance, not hearing).
 The inner ear has two parts that help the body determine its position in space relative to gravity; 1) the semicircular canals and, 2) the vestibule.
 There are three semicircular canals that are aligned at right angles to each other. These canals are filled with fluid and are lined with a nerve-filled, crystal-encrusted membrane that transmits information to the cerebellum (the part of the brain that controls balance and coordination). The cerebellum collects information from visual stimuli and the nerve endings in muscles associated with proprioception (the perception of movement), to assist the brain in assessing where the body is in relationship to gravity and its surroundings.
Normally, when the head moves, fluid in the semicircular canals shift, and that information is relayed to the brain. When the head stops moving, the fluid stops as well, and that information is relayed to the brain.
In some cases there may be a short delay in the transmission of this information to the brain. For example, when a person rides on go-round or spins quickly around in circles, the fluid in the canals develop momentum and even though the body stops spinning, the fluid in the semicircular canals may continue to move. This causes vertigo, or a sensation of spinning, and may cause the person to fall or stumble. It also may be associated with vomiting.
•Acoustic neuroma: This is a benign tumor of the ear that can present with vertigo.
•Vertigo is the sensation of spinning and may present without warning and be associated with nausea and vomiting. Patients with inner ear problems may be unable to move without generating symptoms.
The mild case can be relieved by closing one’s eyes, while the serious case has an illusion of bodily movement with rotatory sensation like sitting in a sailing boat or moving car, and even accompanied by nausea, vomiting and sweating.
Pathogenesis ;
Hyperactivity of the liver ;
The liver is analogize as wind and wood, characterized by movement and ascending. Over contemplation, anxiety, depression or anger can damage the liver yin, resulting in hyperactivity of liver yang. Dizziness occurs in case the liver yang moves as the wind and ascends to attack the brain. Or the kidney water, generally in deficiency, fails to nourish the liver. Dizziness occurs in case the liver is lack of nourishment, which leads to hyperactivity of liver yang. In both situations there is deficiency in the lower but excess in the upper part of the body.
Main manifestations : Dizziness aggravated by anger, irritability, flushed face, red eyes, tinnitus, bitter taste in the mouth, dream-disturbed sleep, red tongue proper with yellow coating, string taut, rapid pulse.
Analysis : Anger damages the liver yin causing hyperactivity of the liver yang which transforms into fire. When the fire flares up, flushed face, red eyes and irritability appear. The spirit stored in the liver is upset by disorders of the liver, then dream disturbed sleep occurs. The red tongue proper with yellow coating, bitter taste in the mouth, string-taut, rapid pulse are the signs of yin deficiency resulting in fire hyperactivity.
Deficiency of blood ;
the heart and spleen are damaged by overwork and over contemplation in case of a weak constitution after a disease. The damaged spleen fails to produce qi and blood, leading to deficiency. In case the brain in poorly nourished by blood, dizziness occurs.
Main manifestations : dizziness accompanied by pallor and lustreless complexion, weakness, palpitation, insomnia, pale lips and nails, lassitude, pale tongue proper, thready and weak pulse. Dizziness occurs mostly after a serious disease or loss of blood and is aggravated by overwork. Loss of consciousness happens in sever cases.
Analysis : Dizziness is inevitable because deficiency of qi and blood fails to nourish the brain. The heart dominates the blood and is manifested in the complexion. The spleen dominates the transportation and transformation to manufacturer qi and blood. If the heart and spleen are injured, the blood will be insufficient, thereby the complexion is lustreless and the lips and the nails are pale. Deficiency of blood leads to palpitation and insomnia. Deficiency of qi gives rise to weakness, lassitude, anorexia and is aggravated by overwork. Pale tongue and thready, weak pulse are the signs of deficiency of blood.
Interior retention of phlegm dampness ;
In a person with generally abundant phlegm dampness, irregular food intake and overwork damage the stomach and the spleen, impairing their function in transportation and transformation and leading to production of dampness and phlegm. Then the stagnant phlegm and qi may impede the ascending clear yang and the descending of the turbid yin, and thus dizziness occurs.
Main manifestations : Dizziness with a heavy feeling of the head and suffocating sensation in the chest, nausea, profuse, sputum, anorexia, somnolence, white, sticky tongue coating, soft, rolling pulse.
Analysis : Dizziness with a heavy feeling of the head is the sign of the pure yang disturbed by phlegm dampness. Suffocating sensation in the chest and nausea are caused by qi obstructed in the middle jiao. anorexia and somnolence are due to the spleen yang deficiency. White sticky tongue coating soft and soiling pulse are the signs of phlegm dampness.

STROKE – wind, considered to be an emergency may be manifested by falling down in a fit with loss of consciousness or hemiplegia, with the symptoms such as slurred speech, deviated mouth. It can be categorized by abrupt onset with pathological changes varying quickly like the wind, from which the term ‘wind stroke’ comes. The Wind energy itself is not a painful symptoms as pain is result of the contraction. When wind is mild, predictable, it belongs to Yin, can be considered Yin Wind Stroke.  But, when wind is strong, unpredictable, short in duration like a typhoon or tornado, it belongs to Yang, accordingly can be differentiated.
Pathogenesis;
Stroke – wind, often occurs in the aged who are in poor health, with deficiency of vital energy and blood, or deficiency in the lower part of the body and excess in the upper part. It may be caused by deficiency of the kidney due to sexual indulgence or by irregular food intake, which impedes the transportation and transformation function of the spleen, leading to production of phlegm from accumulated dampness and transformation into heat. It may impedes imbalance of Yin and Yang. Other causative factors are exasperation, agitation, alcohol indulgence or overeating, over strain and stress or invasion of exogenous pathogenic wind, all leading to upsurge of Yang liver and heart fire which makes vital energy and blood go upward together with turbid phlegm, disturbing the mind and resulting in this disease. In mild cases there are only symptoms showing dysfunction of the meridians and collateral’s, while in severe cases both dysfunction of organs and meridians are manifested.
Tense syndrome results from disturbance of the mind by the phlegm heat or collection of excessive fire in the heart and liver while flaccid deficiency syndrome results from deficiency of the vital energy or collapse of the kidney yang. In untreated or improperly treated cases, the tense syndrome tends to become flaccid and the prognosis is often poor.
Differentiation;
Tense syndrome –  Main manifestations: falling down in a fit with loss of consciousness, tightly closed hands and clenched jaws, flushed face, coarse breathing, rattling in the throat, retention of urine, constipation, red tongue with thick yellow or dark grey coating, string-taut, rolling and forceful pulse.
Wind stirred up by upsurge of liver yang sends vital energy and blood upwards, which together with the accumulated phlegm fire disturb the mind, leading to sudden loss of consciousness with tightly closed hands and clenched jaws, flushed face, coarse breathing, retention of urine and constipation. Excessive wind phlegm brings about rattling in the throat, red tongue with thick yellow coating or dark grey coating, string-taut, rolling and forceful pulse are the signs of wind combined with phlegm fire.
Flaccid syndrome – Main manifestations: falling down in a fit and sudden loss of consciousness with mouth agape and eyes closed, snoring but feeble breathing, flaccid paralysis of limbs, incontinence of urine, flaccid tongue, thready, weak pulse and in severe cases cold limbs or flushing of face as rouged, fading or big floating pulse.
Severe weakness of primary vital energy,separation of yin and yang and exhaustion of energy indicates mouth agape, eyes closed, snoring but feeble breathing, flaccid paralysis, and incontinence of urine. flaccid tongue and thready weak pulse suggest the deficiency of blood and prostration of the kidney yang. If complicated with cold limbs, flushed face, fading or big floating pulse, it is a critical case, indicating exhaustion of yin in the lower portion of the body and upward going of the isolated yang.
There are two categories. One is that only the meridians and collateral’s are attacked without the organs being involved. The other is that after wind stroke the functions of the affected organs have been restored, yet there exists stagnation of vital energy and blood in the meridians.
Main manifestations: Hemiplegia, numbness of the limbs, deviated mouth, slurring of speech, accompanied by headache, dizziness, vertigo, twitching of muscles, red eyes and flushed face, thirst, dryness of the throat, irritability, string taut and rolling pulse.
Analysis: wind phlegm enters the meridians due to imbalance of yin and yang, causing retarded circulation of vital energy and blood. Hence appears hemiplegia, numbness of the limbs, deviated mouth and slurring of speech. It complicated with upsurging of liver yang and upward disturbance of wind yang, the symptoms are headache, dizziness, vertigo and twitching of muscles. If there is excessive fire in the heart and liver, there may be red eyes and flushed face, thirst, dryness of the throat and irritability. Stagnation of wind phlegm leads to a string taut and rolling pulse.

BREAST CANCER
• In its early stages, usually it does not have any symptoms.  The diagnosis of breast cancer has a profound emotional impact because of fears and it affects a part of the body intimately linked with a woman’s self image and sexuality. As a tumor develops with the following signs :
• A thickening or lump in the breast or armpit usually observed after the menstrual cycle; often the first apparent symptom of breast cancer, breast lumps are painless, although some may cause a prickly sensation. Lumps are usually visible on a mammogram long before they can be seen or felt.
• Swelling in the armpit.
• Although lumps are usually painless, pain or tenderness in the breast can be a sign of breast cancer.
• A noticeable flattening or indentation on the breast, which may indicate a tumor that cannot be seen or felt.
• Any change in the size, shape, contour, texture, or temperature of the breast, like dimple in the skin, a reddish, pitted surface like the skin of an orange could be a sign of breast cancer.
• A change in the nipple, such as an indrawn or dimpled look, itching or burning sensation, or ulceration; scaling of the nipple is symptomatic of Paget’s disease, a localized cancer.
• Unusual discharge from the nipple that may be clear, bloody, or another color. It’s usually caused by benign conditions but could be due to cancer in some cases.
• Change in the color or peel of the skin around the nipple.
• A marble-like area under the skin.
• Dilated veins.
• Hardness of the breast.
• An area that is distinctly different from any other area on either breast. One or both breasts develop an abnormal lump or persistent pain, or look or feel abnormal. The cause often is something other than cancer but should be identified.
• There may be swollen lymph glands in the armpits. Any such swelling could be associated with cancer.
It begins with the formation of a tumor. If the tumor is malignant, it has the potential to grow and spread or metastasize to different parts of the body through the blood stream or the lymphatic system.  Almost all breast cancers begin in the glandular structures in the breast that, during lactation, produce milk. These mammary glands are under the control of reproductive hormones that stimulate the monthly cycle of gland expansion and shrinkage, which is a feature of the regular menstrual cycle. Many of the factors associated with the development of breast cancer appear to have their effect through interaction with the hormonal stimulation of these glands. It can be ‘invasive’ (spreading) or ‘noninvasive’ (do not spread and is confined to the original site). There are different kinds of breast cancer;
 Ductal carcinoma in situ(DCIS),  Lobular carcinoma in situ(LCIS),  Invasive(infiltrating)ductal carcinoma(IDC): This kind of cancer begins in the milk passage, breaks through the wall of the passage and spreads in the breast tissue. Later the cancer cells spreads to different parts of the body. Invasive (infiltrating) lobular carcinoma (ILC): This condition begins in the milk lobules and spreads to different parts of the body.
 Inflammatory breast cancer(IBC): This is a rare invasive condition where a tumor or lump is not seen,instead the skin of the breast looks red and warm to feel. The skin is thick with pitted appearance and looks like an orange peel. The breast becomes larger, firmer, tender and itchy.
 Diagnosis is based on detection and screening. It can be done through self-examination of the breast frequently and finding out for the presence of lumps, pain in the breast, thickness, changes during menstruation. Getting checked with a physician is necessary if anything unusual is observed. Self-examination helps in early diagnosis of the disease.
 But if one has already have the Chemotherapy, it may develop some side effects which causes nausea, vomiting, infertility problems and early menopause by destroying the eggs in the ovaries.
 Hormonal therapy causes vaginal dryness and hot flashes after menopause. Hormonal therapy: Those who have undergone menopause and taken hormone therapy for more than 4 years with estrogen and progesterone have high chances.
Herceptin can cause heart problems by weakening the heart muscles.
Side-effects of radiation therapy include swelling in the breast, reddening and blistering of the skin and fatigue.
One should avoid long-term hormone intake after menopause.  Breast-feeding will lower the risk.  Conceiving before 30 years of age.  Loosing extra kilos, as extra fat increases estrogen production.  Consuming healthy food with plenty of fruits, vegetables and whole grains.  Exercising regularly.  Decrease alcohol consumption.
The chances are high as the age increases or if anybody in the family has suffered from breast cancer indicating genes are responsible. If one has breast cancer in one of the breast, there is a likelihood of getting affected to the other breast too. Chances of recurrences are high to the same part or to different parts of the body.
Menstruation: Women with menstruation beginning before the age of 12 years and menopause occurring after the age of 55 years are at high-risk of getting affected with breast cancer.
Having higher estrogen levels increases the risk.

The spine is a very complex structure that is never still, and it is involved in the slightest movement, including breathing. The main source of pain and stiffness is the protective spasm of muscles around the injured part. The treatment for acute symptoms of uncomplicated back strain consists of rest. If one feels pain while on rest or if it is severe enough to restrict most activities or there is no improvement in a week’s time, it needs more investigation. Chronic back pain associated with other symptoms such as fever should always be investigated as it may be a sign of disease of the vertebrae or spinal joints and occasionally may be caused by disorders of the abdominal or pelvic organs particularly the kidneys and reproductive organs.
Spinal nerves have motor fibers and sensory fibers. The motor fibers innervate certain muscles, while the sensory fibers innervate certain areas of skin. A skin area innervated by the sensory fibers of a single nerve root is known as a dermatome. A group of muscles primarily innervated by the motor fibers of a single nerve root is known as a myotome. Although slight variations do exist, dermatome and myotome patterns of distribution are relatively consistent from person to person.

Each muscle in the body is supplied by a particular level or segment of the spinal cord and by its corresponding spinal nerve. The muscle, and its nerve make up a myotome.
One should follow the followings when driving long distances;
 • Travel slowly over rough roads.
 • Ensure that the vehicle has well padded seats, which will help reduce the amount of vibration being passed up the spine.
 • Support the lower back with a small cushion or lumbar roll.
 • Tilt the seat backwards to about 110 angel to reduce vertical pressure on the spine.
 • One should change the posture frequently to encourage circulation and prevent muscle tiredness.
 • Make sure that the distance from the seat to the foot pedals suits your legs.
 • Take heavy objects out of back pockets to reduce twisting forces on the lower spine.
 • Take care when getting in and out of vehicles. Sit sideways on the seat first then lift your legs in and rotate your body into the travelling position and do the opposite when getting out. Avoid sudden bending or twisting movements.
 • Reduce driving time. Frequent stops help. As often as possible get out, stretch your back and walk around a little.
 • Exercise regularly to help strengthen lower back muscles.

 -Pain in the small of the back may indicate kidney disease such as pyelonephritis.
 -Lower back pain and stiffness may be caused by muscle, joint, ligament or disc injury.
 -Pain and tenderness at the tail end of the spine can result from injury to the coccyx from falling onto the buttocks.
 -Sciatica, caused by pressure on the sciatic nerve is pain in the buttock that spreads down the back of the leg to the foot.
 Reducing back strain;
 
-When lifting heavy objects, keep your back straight and  bend your knees. Do not stoop, if the object is too heavy, ask someone to help.
 -At the desk, keep your back straight and rest both feet on the floor. A well designed office chair provides good back support.
 -Your load should be equally distributed if possible when you are carrying bags.
